Since there weren't that many people on the planet until about the 19th century onward, even though the scope of your question is huge, the area from which the correct answer can come from is small.
The top three, I would think would be:
1: Josef Stalin (about 30 mil)
2: Adolf Hitler (11 mill to 27 mill depending on what you blame him for)
3: Pol Pot (unknown, but not 10 mill)
