Adobe Acrobat/footer text & page # in PDF do not print Adobe 8 Standard

Question
1.0  Footer created using Adobe 8.0 Standard toolbar does not print.  Nor does it appear in the "Preview Composite" box of Print dialog box.
2.0  PDF doc has 40 pages in one continuous file.  
3.0 Started with a single two page PDF.  I printed the web page using printer called "PDF Factory" to a file.
4.0  O/S is Windows XP PRO.
5.0  Opened the PDF document in Adobe 8.0 Standard.
6.0  Used toolbar:  Document>Insert pages> to add another PDF document consisting of three pages.  
6.1 Now PDF document has five pages.  Saved document
6.2 repeated steps 6.0 and 6.1  multiple times. Document now has 40 pages.
7.0  Used toolbar:  Document>Header& Footer>Add
7.1  Set two line description in footer text left.
7.2  Set page # format to "Page n of 38"
7.3  Inserted page # in right footer text.
7.4  Top and bottom margin settings 0.5" and 0.5".
7.41  later tried Document>Header& Footer>Update to 0.8" no difference.  Did not print footer.
7.5  Checked both boxes in "Appearance Options".  
7.51 Shrink document to avoid overwriting the document's text and graphics.
7.52  Keep position and size of header/footer text constant when printing on different page sizes.
7.53  Saved settings.  Clicked OK.
7.6   Set "Page Range Options"... start print on page 3.  All pages in range 3 to 40.
8.0  File>Print then select... page 6.
8.1  No footer appears in "Preview composite" box on the Print Dialog screen.  Even though the footer does appear in the continuous PDF file document on screen.
8.2  Footer does not print on hardcopy of Page 6.

Hi Neil,

I have tried to recreate your problem without any luck. Try switching fonts, it is possible that the font you are using is only for display purposes and will not print due to licensing.

I am not familiar with PDF Factory, it is possible that something with the way the pdf was created is keeping you from adding/printing your footers.
