The last response assumes you don't mind creating a new form. I have a similar question in how to have todays date be the default in a .pdf that you want to add some fields to or already has a date field. Additionally perhaps the form was originally created in Word and printed to .pdf and then the fillable fields added. So the question remains unanswered at this point.
