The prevention of STDs is very high priority subject in the Adult Film Industry. Thanks to modern medical technology and a higher standard for testing in Adult Business, the odds of a performer contracting an STD is actually very low. In fact, you are more likely to contract an STD from a cheating spouse than you are working in the Adult Film Industry
.
Performers are tested at reasonably regular intervals, and the frequency of testing varies with the amount of work that a performer actually does. As a general rule, most producers insist that the test results on hand at the time of a shoot are no more than 30 days old.

You may have also heard STD testing referred to as an AIM test. AIM refers to the Adult Industry Medical Healthcare Foundation. AIM is an organization that provides specialized STD testing to Industry professionals (as well as the general public) and has set guidelines for testing that are now widely accepted as the Industry standard. The organization was founded by Dr. Sharon Mitchell who, oddly enough, is a former adult performer and an AVN hall of fame inductee, thus making her extremely dedicated to the task of keeping Adult performers healthy.

Now for the second part of your question.
Yes, female performers do have a great deal of control when it comes to who they work with and working with males who have an unusually large penis is by no means mandatory.  While many male performers are a bit large than average, not all of them are packing “monsters” in their pants. The actual average length is somewhere in the neighborhood of 8 to 9 inches.
If you are serious about becoming an Adult Performer, I would suggest you start by contacting a few reputable Adult Talent Agencies and speaking to them about your interest in the Adult Entertainment Industry. I recommend you start with these:

These are reputable Agencies and their websites have all the information necessary to put you in contact with a representative. These Agencies only work with equally reputable Production companies thus greatly increasing your chances of success. If this is something you are serious about, get yourself signed with an Agency and let them get you started on the right track.

For your own safety I would also STRONGLY advise you to avoid placing and/or answering ads on sites like xpeeps.com, craigslist.com and backpage.com looking for work. These, and many similar, sites are common playgrounds for all manner of predator and are honestly never used by legitimate Adult Industry Professionals to any real extent.
