What forms and agreements do I need for models and crew.

Answer
You'll need a 2257 compliant model release, which contains terms applicable to the type of content you're looking to create and the ways in which you want to distribute it.  (Depending upon your jurisdiction, this can take different forms.)  You'll also need independent contractor agreements for your crew (assuming they are not employees of yours).

Don't forget that you'll need to comply with the record keeping and labeling requirements of 18 USC 2257 and 18 CFR 75 for your models.
