I am not sure what you mean by 18 U.S.C 2257 CFR title 28 part 75 forms.  These federal laws and regulations required producers to collect and maintain specific identification records for talent used in content that contain actual or simulated sexually-explicit conduct.  They also require that producers label the content in a specific way to identify compliance with these regulations, and to provide the identity of the custodian of the records.

If you are referring to model releases which require the talent to provide identification and information sufficient to comply with the federal regulations, I have seen a few floating around the web, but none that I would personally suggest using.  There are many requirements and exceptions in the regulations, and attempting to go at it on your own without professional legal guidance is extremely risky.
