what happens when a patient comes to consciousness during surgery?

Answer
They don't come to consciousness during surgery. That's the point of anesthesia. If they start to emerge from the anesthetic, their breathing, HR and blood pressure change and the anesthesiologist recognizes that and gives them more anesthesia.
