Question
I've the following questions:
1. In Islamic countries (and in which countries), is Jizya tax imposed on non-muslims?
2. In Islamic countries, If a person converts, what is the punishment?

Answer
In the Middle East and other Islamic countries laws vary. Only two countries in the world currently adhere 100% to Sharia law, Iran and Sudan. There are other countries where there are "pockets" of Sharia law enforced (i.e. Afghanistan and Somalia). Iran is the strictest country for enforcement of Sharia.  

Non Muslims are "taxed" in Saudia Arabia, Yemen, Sudan, Iran, and possibly others that I am not currently aware of.

The punishment for conversion is often death in Sharia led countries mentioned above, if not by the government then almost certainly by their families. At the very least even in non-Muslim countries (like Lebanon and Israel) they are disowned.
