I came across a document from Iraq of someone's personal information.  It has listed :Name of surname and the third?  Im not sure what this means.  I speak a little arabic, but am not sure what the name of the third or three means?  Can you help me?

The "triplet name", consists of three names:
1-First name
2-Father name
3-Surname

For example, one Arabic man name can be "Ahmed Khalid Tamimi":
1-"Ahmed" is the first name, which we use to call that man.
2-"Khalid" is his father's name.
3-"Tamimi" is his surname, which is usually his grandfather name or his family name or his tribe -clan- name.
