Try using Lavender oil (massage oil from Bath & Body Works) it is anitbiotic, antiseptic, antiviral and antifungal.  If used in water, it should dilute it some so it shouldn't hurt the dog if they lick it.  I've used it to help with my dog's ear infection.  I find it to be a wonder oil.

My dog has the same thing going on. I have been to the vet many times and have done 2 oral antibiotics as well as steroids to no avail. My dog is a german shepherd as well. The vets only thing that she can come up with it being is unfortunately Nodular Dermatofibrosis. Here is a link to some good information on it.

I really hope it isn't that but it sounds like it could be. My dog is 8 years old as well and that is the age where it starts to show signs.
