I am trying to find and application, or website, that will proivde sun and moon azimuth and elevation angles, by location (latitude, longitude [, and elevation, if possible]).  Do you know of such an application or website?

Hello,

There are two (software) apps I know of that will probably address these issues. They are:

'Astronomy Lab' and

'CyberSky'

Go to google.com and type each of these into the search field, and you ought to be able to locate the respective sites where they can be downloaded.

I believe you can obtain shareware copies of each, and - if you like them - pay the amount designated for the complete program.
