Question
First of all, thank you.

I have a 20 month old nephew who is just now starting speech therapy because he will not talk. He says no words at all.

I am wondering if he is autistic. He not only has no words, but he does not point, will not point out pictures in books, gives very, very poor eye contact, has never played peekaboo, will not smile back at someone, will not respond to his name when called, and never, ever engages in any play with his sibling or cousins. When the room is full of children his age, he is off doing his own thing, never interested in the kids or what is being played with.

Outside, he spends all his time picking leaves off bushes, tearing off branches, and walking around with sticks. He will not engage in playing with balls, chasing balls, etc. He loves carrying around brooms.

He will push people over to things when he wants something, but otherwise has no interest in communicating. I sat beside him in a restaurant for over an hour and not once would he respond to anyone talking to him (not even by making eye contact). He seemed totally absorbed in his food and had no interest in ANYONE around him.

He does not spin or flap. He does not line things up, but he is very advanced at stacking blocks.

At first I wondered if he wasn't deaf, but now I have suspicions that he is autistic, though his parents don't seem concerned.

Do you think he is, and if so, will those doing his speech therapy perhaps discover this as well? I hope so, because I think he needs intervention. I am worried about the little guy, and I am sad to see that I am the only one suspecting anything.

You can try to share these sources with his parents and hopefully his speech pathologist will also recognize the signs.  Hearing from a "professional" versus a family member can sometimes prompt parents to be more receptive.  The speech therapy should focus on creative play to increase functional communication if possible.  The following websites may also be helpful:
