I would say don't be concerned. The point of ignoring your child's statement is to show him that when in session it should be just him and the therapist. Responding to an irrelevant statement is giving into the power control between therapist and child. The therapist must be in control. Otherwise your child would not learn. The temper tantrum should be ignored. It's only week three, he needs to learn that only through good behaviour will he get positive appraisal and attention. There should be no concern, his vocabulary seems fine as long as it's shaped to suit social standards. The therapist should praise relevant language attempts and reform irrelevant ones.

Check out Floortime/DIR therapy. Playproject.org is a good place to start.
This therapist is very disrespectful of your child, as much of ABA is--for young children, especially. I would try DIR first and if you don't see improvement, try the ABA when he is a bit older.

IBA or ABA are methods of fixing a person who is not broken. I realize this is a year after you wrote this, I hope you have gotten better support. You answered most of you questions. I recommend looking up Amanda Baggs on you tube. Very good resource for your future journeys. I have learned and applied it all.
