First of all, please be aware that the advice that I offer here is not based on experience rather than actual medical knowledge.  I'm not a doctor.

My Master and I don't usually engage in this kind of play, as our relationship is based more on service rather than the giving or receiving of pain.  But I did speak to a friend of mine who does enjoy anal play.  One risk that comes with any use of a butt plug is hemorrhoids.  But you should be able to use one for an extended period of time without serious complications, depending on how much pain you can tolerate and enjoy.  You should, however, use lubrication when removing the plug as well as when inserting it.
