Ballet/made to fit pointe shoes

Question
When I was 17 my teacher had me send an outline of my foot with marked location of metatarsals to a company in San Francisco that made my shoes to order.  They had a fiberglass box.  I now have a student that would benefit from a shoe made for her rare shaped feet.  Any ideas who this company is??

I viewed a couple of forums and it seems Capezio is easy to deal with - but I'm not sure how up to date all these links are.

None of these mention the fiberglass box particularly.
