Question
How do we recind the reaffirmation agreement?  It is within the 60 day period.  Is there a form available?

There might be a form you can find online,
otherwise just put the legal caption at the top, entitle it rescission of reaffirmation agreement

and type a paragraph that includes the date you are filing it, the name of the creditor and reference the date the reaffirmation was filed.
You should have a certificate of service and a service list attached.  You should file with the court, and send copies to the creditor, the creditors lawyer (or whomever you dealt with for the reaffirmation) and the company's registered agent in the state if you can find that out.
