"ordinary effort" is the key as well when judging a play where the defender has to make an EXTRAORDINARY effort to get to the ball, and then is unable to make the play.   just because a defender manages to get a glove on it, and fails to make the play, does not mean it should be scored an error!   MLB rules lean toward the batter, and generally a hard hit ball is scored as a "hit" unless the defender makes the out.
