Put simply, a mutation is a change in the DNA sequence. There are several kinds:
Deletion - a nucleic acid is left out
Insertion - a nucleic acid is added
Frameshift - a change in the 3-codon reading frame
This sounds like a homework problem to me, so answer the rest for yourself
