Question
How do you determine if a foot lesion is caused by a focal infection of an organism or a systemic infection? is there any disease that cause lesion only on the foot skin?

A bacterial infection would have pus.  If it is a systemic infection other parts of the body would be involved. Some fungal infections can be on the foot only.  A lesion on the foot could be a neoplasm and not a bacterial on a fungal infection. I base my answers on my experience as a microbiologist but I am not a medical doctor so if you are inquiring as a patient I cannot give advise about to do about it.
