Dear Mr. Goldberg,

Three years ago, I was evaluated by you and your team at the Presbyterian Hospital, in order to enroll in a free program for treatment and research of Manic-depression  I have manic-depression and an eating disorder.  Unfortunately, I did not continue the process at that time.  I also lost all the contacts and phone numbers I had from you and your team.  I would like to know if I can come back to the treatment.

While I was affiliated with the Columbia-Presvyterian Medical center for about 30-years, I left there in the early 1990s.
