Question
I have been on the pill for 12yrs now.  I have only had one partner, my husband of 10yrs.  We always use the pill and a condom if he is going to climax inside me.  We really want to have sex without the condom but do not want children at this time in our lives so are very paranoid.  How effective is the pill after 12yrs with never having ejaculate inside me if we choose to do so?  Should we use spermicide if we do not use the condom to be safe?  

Answer
The pill is very effective as long as you take it every day.  Out of 100 couples using the pill for one year, less than one woman will get pregnant.  So if your husband ejaculates in you every time, but you take the pill and don't miss any, there is less than 1% chance that you'll be pregnant in 1 year.  Best regards.
