I think the heart attack brought about weight gain indirectly. It made him quit smoking, which although healthier, is probably the reason for his weight gain

Bowie's appearance at Fashion Rocks was a reflection of the events which had occurred during Hurricane Katrina. The "flooding" pants he was wearing represented the major flooding which was occurring in the south. His black eye represented the fighting which was going on in the south.

I know this is a late response! But, actually, this benefit concert (Fashion Rocks in 2005) took place only days after the flooding of Hurricane Katrina. It was a somber time for all who watched or endured the unbelievable devasation of New Orleans. Bowie came out on stage in flood length pants, a bandaged arm and painted-on black eye, intentionally, to honor the sufffering and the dead because they were (or should have been) on all of our minds that evening. Even while we watched such a fantastic musical performance, he was reminding us, with his appearance, of a MUCH greater, more important life event taking place else where. He's truly amazing....so purposeful in his choices. Even his songs that night seemed to fit the mood perfectly...Life on Mars & Five Years.
