Question
Why is it so hard to transfer practise results to actual league
or tourney play?
I, practise my spare shooting and target angles,break points and
adjustments but on league day or tourney day, all of this work
leaves me with meager results. What is the problem and how do I fix it? I, practise 15 to 21 games a week. I have a 158 league average
but average 170-186 in these practise sessions.

If possible, try to find a coach to work with.  By doing that, you can maximize your practice and make sure you are practicing the correct things as well.  You might find out you can improve with less games per week as well.
Working with a coach will also make sure your practices have a good pace.  This might be part of your problem.  Practice is usually at a much faster pace than league or tournament play.  Also the lane will break down differently and that can make spare shooting different.
If you practice well, then try to have the same outlook when bowling league or tournaments.  Placing too much importance to the scores during your league play can make you play tense.  That will affect your shot making ability.
Another thing to try is to not think once you step on the approach.  Plan your shot before you step up on the approach and once there, just do it as they say.
