How can i get a license to host a boxing match in the state of Arizona?

Individual states have unique requirements for securing a promoter's license, so you'll want to check directly with Arizona's athletic commission.
Here's a link to the commission's page where you can locate the license application and fill it out. It says the promoter's fee is $200:
