Question
Hi Doctor, I recently had a lumpectomy for high grade DCIS left breast. I had an MRI before the surgery.  
Impression: Corresponding to the biopsy-proven cancer in the upper inner  quadrant of the left breast there is on MRI a bi-lobed malignant enhancing mass with maximum craniocaudal dimension of 9mm
Extensive background enhancement in both breasts. Enhancement in numerous foci. These measured 4mm or less in size. Difficult to characterize these foci further. Enhancement may be related to benign process however foci of DCIS cannot be ruled out completely. On mammography numerous calcifications are present throughout both breasts.
Also on the report,  Extensive micronodular background enhancement is diffusely present. The largest nodule measured 4mm in size. Rapid inflow with persistence curve is present.
What does all this mean?

Answer
The important thing here is that there is a risk of several other foci of cancer in situ (cancer stage 0) in both your breasts. I would suggest more MRI guided needle biopsies of these lesions - at least the larger ones - in order to find out what they are. If indeed there are several such foci in both breasts a bilateral mastectomy must at least be considered. Please do keep me posted!
