Breast Cancer - Suspicious Breast Lump on MRI

Expert: Claes-Gustaf Nordquist, M.D. - 6/19/2009

Question
I recently had a breast MRI, a month prior I had a mamma and an ultrasound.  I am 45 y/o and have very dense breasts.  I have had a few benigh lumpectomies in the past and in my left breast I had my milk ducts removed just to give you a little history.  My question is:  On my recent MRI there is in the right inner upper quadrant an enhancing mass measuring 9x6x6 mm.  The mass demonstrates rapid enhancement to one minute followed by a washout phase.  The enhancement is heterogeneous.  The mass is well defined with microlobulation.  Is is suspicious based upon the enhancement pattern and it's heterogeneous enhancement.  Can you please let me better understand what the MRI prognosis means.....heterogeneous, microlobulation.  Can you give me an idea of what I could possible be dealing with?  Thank you very much.  I am scheduled for an ultrasound guided core biopsy.  Thanks again.

Answer
You are giving me a tough order. I can hardly give you a crash course in magnetic resonance imaging (MRI) diagnostic radiology. But in order to keep it short and not too technical it means that what is seen shows different signal levels in different parts. Microlobulation means that it is divided up in SMALL lobes. It is the heterogenous enhancement and its enhancement pattern that looks suspicious. However we can NOT be sure what this is until we have the result of the biopsy. So your biopsy is very important! Please do keep me posted!
