Building Homes or Extensions/floor load ratings

Question
What is the dead, live and concentrated load ratings for a raised floor constructed of 3/4" 5-ply plywood decking layed on 2" x 4" No. 2 Hem Fir joists. The maximum span at any given point is <5feet.

Tim, this is a strange way to approach the question.  I like the span calculator here:

which will take you to a solution that works for your scenario.  I used the choice of 16" centers for the joists which will give 60 lb/SF.  By increasing to 12" centers, this loading can go  to 80.
