Question
should the contract to install a concrete driveway include a start and or completion date?

Answer
I don't know if you are speaking as a contractor or as a home owner, though the issues are similar.  Concrete work is very subject to weather.  If you want to include dates use language similar to this:

Work to begin on or about xxxxx, subject to weather conditions
work expected to take xxxxx, again subject to weather.
The price indicated on this quotation is good until xxxx.

No one wants concrete poured in freezing temperatures.  Saturated, muddy subsoil makes it difficult to work, lengthens the job, reduces the quality of the finish product.
