Question
I was working at a hospital for 2 years when I started having family problems. I missed a couple of days because of it and told my boss what was going on. Then one day it got so bad I had to call my boss and quit. She offered me to seek medical advice so that I might be able to go on medical leave. I couldn't take it and told her it ok I will just give my resignation today. She told me she understand and to write a resignation letter to keep in the hr file, so that if I want to apply in the future they will see that I resigned because of personal reason. She said if I do try to re-apply that I can use her as a reerence.
3 years past and they are hiring again for same position. I want to re-apply but I also want to email her a letter that I'm ok and that I would love to get my job bacK so that she can let HR know to look up my application online.

Dear Christina,

  Please do go ahead and try to get the job back. Summarize briefly the past events (Even
if you are sure they will remember you. Just in case they have forgotten details.) and then
explain why everything is fine now and how you do not foresee any such break in your employment
record again. Talk about what you did for the hospital (any plans/programs/awards) that made
you the sort of employee that they would like to get back. It would be helpful for them to be
reminded of what is so special about working at X hospital as well. In other words, make them
know that this is more that just a job opportunity,you really do value them as employers.
