Business Software/MS ACCESS CHECK BOXES

Question
I am trying to set up a form with check boxes.  I would like to be able to check the box on the form so when I run a report from it I get the heading of that check rather than the actual check box (see example 2).
EXAMPLE 1
Contract number 1   blue  yellow       

Contract number 2   green

Contract number 3   white

as opposed to current set up :

EXAMPLE 2
                   Green Yellow White Blue
Contract number1             x          x
Contrat number 2      x        
Contrat number 3                   x



Currently the only thing I can do/get is one colume per heading with multiple check boxes under each heading.  This is to dificult to read (example 1).

Answer
You might be able to get the result you are looking for by having each color be a separate field. Then a "check" on the form could be recorded as an "x" in that color's field for that record.

For the most part I have been working in Open Office rather than MS Office. In addition, my work with databases is limited.
