i want to relist my item. Can i add another item in relisting while i didnt place any second category while listing my item for the first time?

I'm not sure I understand your question.  You cannot add another item to an auction you are relisting. It should be for the same item that was in the original listing.  

You CAN, however, add another category, even if the first auction was only listed in one category.  when you click the relist link on MyEbay, you will see the auction in sections. One of these sections is titled "Second Category" and the link across from it says "Add a Second CAtegory".  click this and select a category just as you did for the first category.
