My husband left for basic training a couple of weeks ago. I do not have DD Form 1172 that I need to get my military ID and for insurance purposes for me and our son. Do you know who I need to contact or how I will get this form from my husband?  Obviously this is not something I can just call him up and ask him about.  Thank you for your help.

You do need to get the DD form 1172 from your husband.  If he refuses to sign one and send it to you, you need to contact his chain of command.

Please let me know where he is stationed, if he does not send you the form, so I can send you contact phone numbers or e-mails to get this resolved.
