Question
I was medically discharged from the Army in1999. I have since had
another surgey. And doing better, I want to come back active duty. I would
like to know if I would be able to come back and would I have to take the
ASVAB and go through basic training again.

I cannot answer your question as written.

Whether you can return to military service depends on your discharge classification, your re-entry code, and whether your injury and repair surgery are waiverable.

Since you have been out of the service for 10 years, you would have to re-qualify as an enlistee and, if accepted, would have to take basic training again.
