Just wanted to know if is was possible to re enter the navy after being
discharge for BCA/PRT failure if you correct the problem with 90days of
the official discharge. 12 years in, no NJP nor any prior issues other than
BCA failure (2).

Most likely not.  Body Composition Analysis/Physical Fitness Assessments are taken very seriously in the Navy.
Usually it takes 3 failures before you are discharged.  If you corrected the problem within 90 days, they'll wonder why you couldn't do that a long time ago when you were still in.

In your discharge paperwork find your DD Form 214.  Down at the bottom in block 26 is your Separation code, and in block 27 is your ReEntry code.  Your RE code will tell you exactly if you can reenlist in the Navy, or any other Service.  See your recruiter soon, because the yearly quota for reenlistees goes fast.
