Hello,

I am currently serving in the Air Force and my supervisor mentioned this award to me.  I was wondering what form I need to fill out and what at exactly to put on the form for this award.  We are thinking it is the standard for form EPRS or for BTZ  packages but aren't sure.  Thank you for your time.

Hi Eric,  The Air Force Instruction for Awards and Decorations is AFI 36-2803 --THE AIR FORCE AWARDS AND DECORATIONS PROGRAM (see: http://usmilitary.about.com/od/afreg/l/blafi362803.htm).

Table 6-1 (page 34) of the AFI states that one should use the format shown in DoD 1348.33-M, "Manual of Military Decorations and Awards (see: http://usmilitary.about.com/od/fy2004paycharts/l/blperenpay.htm).

The format shown in the DoD Manual is a letter format (see page 62 of the manual).

According to the AFI, the approval authority is the Group Commander.
