I was contacted by a band that wants to use some of my images for their CD booklet. I am charging them a flat fee for the images, no royalties.
This is the first time someone has asked to use my images in this manner...I have only sold physical prints before, so this is all new to me.
I want to create a form for them to sign and agree to, that indicates I am allowing them to use the images as long as it pertains to the band...but I want to maintain the rights to images though, because I want to still be able to sell physical prints of the images....
I must note that the images they are paying for are slightly different in tone and color but are still the same subject manner as the originals I still want to sell personally. I am not allowing them to alter the images...I am allowing them to reprint the images, though not individually for profit.....Basically they can only profit from the images by selling their CD, since the images will be in the CD booklet.
So i guess what I'm looking for an answer to is(forgive me if this isn't entirely clear)....what would be the official license, or rights to indicate the images are under....that describes what I'm allowing them to do with my images.
Also, are there any websites that can help me with creating this form for them to sign? I'm trying to make it as professional sounding as possible without sounding ridiculous.

What you are doing is very common and you can get these forms by joining APA or ASMP. apanational.org is a great site with a wealth of information on the business of photography, contracts, estimates, invoices and the like. There are also books that have these from Allworth press. Another good resource is Blinkbid, estimating and invoicing software, blinkbid.com.
