hello there,
i need to write a letter from a responsible person(eg teacher), confirming information about my date&place of birth, and then give it to him to sign so i can send it off to a college i'm applying to. could u please help me with a structure of this kind of letter?

Such a letter needs only to say:

 According to school records, Nattalie was born on (date) at (city) (state).

And a place for the signature. That's it! Unless there's more you're not telling me, that's all you need. There's not much structure needed to a single sentence.
