Cats/lost my cat 3 months ago

Question
one day my cat ran away because we left the door but i didn't notice so my cat ran out the door but i didn't know it was open after 1 month the cat was found sleeping in my neighbors chair that she has outside on her pouch but when he came back he had a infect that he can't pee in his litter box he would steep in the litter box but he would only drop some drops he kept on peeing everywhere so i bought i leash  so when i go to school he wouldn't pee in and next to the bathtub  but one day i came home from school i saw that the window was open so i check it my cat leash and the collar with his name and address drop on the floor next to the window i saw that the leash was hanging out the window i know that if the cat wants to take the leash off he would use his paws to squeezes his head thought the collar but i don't know if he did that when he jump out the window i been putting fresh water and his favorite cookies outside on the pouch where he can reach but he hasn't cum home yet it's been 3 months. :(
