Chimney & Fireplaces/Firebox repair

Question
I have a fireplace that has the refractory panels on the bottom and three sides. The back panel is cracked and falling apart. Do you know of a place that makes genaric sizes or custom sizes such that I can get a replacement panel? I've tried my local fireplace store, but they said they can not do anything unless I have a make,model and serial number. I can not find this info any where.
