I paid for a new chimney crown 3 years ago.  Should the crown look like this (see attached pic) after 3 years?

Hello Scott, it looks like the crown is lifting it will do that if the Cemented right up the the flue tiles (there should be a 1/2"-4" gap filled with flexible sealant like Silicone) that way when the flue gets hot it flexes and does not lift the crown. While this is not a "Code" it is how a proper chimney crown should be done.
