Dr. Durnin,

I have been experiencing chest pain in the center of my chest and both pecs deep and long lasting.  I am 34 yrs old and run marathons and have been through several tests with physicians and no luck.

I have had a stress test, echo graph, cardio catheter, an upper GI, endoscopy of my stomach for acid reflux.  So far no luck solving this pain that I'm experiencing.  I have had chest x-rays and still no luck of bone fracture and/or lung defect.  

Could I have just torn my pecs doing push-ups or lifting light weights.  Any suggestions would be greatly appreciated.

Hi Steve,

To know for sure if it is cardiac or muscle origin, take a tab of nitroglycerine when you have symptoms, if they go away you have angina. If it makes no difference, it probably is a pectoralis minor insertional problem from overuse. This can be verified by doing butterfly's, if under load the chest pain increases, it is most likely a tendonitis which will require rest and ice for a few weeks. In the mean time, avoid stressing these tissues to give them a chance to heal. A hiatal hernia may also be a possibility, a barium study can rule this out.
