Question
My report say:
1. Normal Lumbar Lordosis is lost.

2. Early degenerative changes with osteophyte formation seen in L4-L5 vertebrae.

3. Mild diminution of disc space shadow between L5-S1 vertebrae.

4. Pedicles, spinous processes & para-interarticularis appear normal.

5. Sacoiliac joints reveal no obvious abnormality.

Kindly advice what is all these and treatement I should follow.

I shall try, but I have a very incomplete picture.

"Normal Lumbar Lordosis is lost."- the usual, normal, natural curve of your low back is lost.  This may result in/from muscle spasm, patient position during Xray testing.

"Early degenerative changes with osteophyte formation seen in L4-L5 vertebrae" -- OsteoArthritic degeneration.

"Mild diminution of disc space shadow between L5-S1 vertebrae"- mild narrowing, mild loss of expected size of disc at L4- L5 level.  Could be due to herniated disc, could be due to OsteoArthritic degeneration.

"Pedicles, spinous processes & para-interarticularis appear normal."- these sections/ parts of the vertebrae of the spine appear normal.

"Sacoiliac joints reveal no obvious abnormality"-- the SacroIliac joints-  we have one on the left, one on the right- appear normal.

....."-  This appears to be an Xray report.  There is no presentation of complaint, age, history.  What treatment would I follow?  From the above (presumed) Xray report, with no chief complaint/ history; I really do not know what to say.

Sounds similar to what I would expect in an Xray report for one of my Staten Island Arthritis patients complaining of low back pain.

Take some Glucosamine, Chondroitin, Calcium/ magnesium/ Vitamin D for nutritional support for the OsteoArthritis.  See a Doctor of Chiropractic for evaluation of the postural changes (loss of normal Lumbar curve), possible adjustment to restore more normal movement and position.
