QuestionDoctor Gillman, I am a 22 year old male who has been powerlifting for about 8 years now. I have had osteolysis in my right shoulder from doing bench press but this issue has seemed to go away some. I recently have had sharp shoulder pain that inhibits me from actually performing bench press due to an intense pain in my left shoulder. I have read up on some things and I'm not sure if it would be tendonitis or not. I think the problem started after doing a set of heavy shrugs a while back. The pain in my shoulder occurs not when lifting my arm over head, but when pulling or extending my arm outward parallel to the ground and especially when extending my arm downward perpendicular to the ground. The pain seems to be concentrated at the end of the clavicle where the rotator cuff tendon is located and also somewhat in the rear deltoid. The pain mainly inhibits me from doing bench presses with weight and shrugs and deadlifts. What could this possibly be?

You may still have osteolysis at the distal clavicle. It could be that simple. You also could have a SLAP lesion (Superior Labrum Anterior to Posterior) where the biceps tendon traverses into your shoulder joint. Either way, your case truly requires direct physical examination first, then determination as to whether you need imaging (MRI) or not. The sure bet is to see an orthopedic doctor, otherwise find a credentialed sports chiropractor. 'Sorry I can't diagnose this over the internet. You started powerlifting at age 14? Wow.
