I would ultimately recommend listening to a qualified Physical Therapist as they typically can tell you what muscles need to be worked. Pilates is another option that can help correct this sort of imbalance.

However, what you describe is exactly the problems that I started having after a neck injury I sustained in college. What I have learned is that your lower traps are incredibly important in achieving proper rotation of the shoulder, and that most people in the US have chronically weak traps as a result of a sedentary lifestyle and too much time sitting with incorrect posture.

My suggestion would be to start working on some lower trapezius exercises. Use low weights, and strict form- if your shoulder starts to drop while working the traps, then stop the set. Over time you'll notice a huge change in your posture.
