Cigars/Mushy Finish

Question
What causes a properly aged and apparently properly humidified cigar to turn mushy at it's finish?

Answer
First what is properly aged?
You kept them in their original box in a refrigerated humidor by Vigilant at 65 degrees and 73% RH for 5 years?
Or?

A cigar that is on the top of a packed box could be too mushy.
Any mass produced cigar has the better chance of having any problem.

What kind of cigar?
How long in the humidor with how many cigars?
What kind of humidification?
In the plastic or not?
