Question
I have a strange coin a 1989D quarter with a very high edge on both sides and about 1 mm smaller in diameter than the normal quarter. the edge is not milled but plain. The weight seems to be normal. The raised up edge is around the entire coin on both sides.

You state the design is well centered on the coin. This leads me to believe the coin was altered after leaving the mint.  Is the rim sharp?
The coin was likely reduced in diameter and the reeding removed. If it had been an undersized blank (not caught by the automatic riddlers at the mint) there is only a very slim chance the coin would strike up well, and on center.

The flow of metal depends on the outside being proper size to fill all the voids in the die.
