Question
If an in-network medical provider failed to fulfill its contract (with network/insurance), ie. to file a medical claim on the timely basis, but instead referred the bill to collection agency, is there a case against the provider?

How to obtain the contract as a proof of  provider responsility?

I have to ask a question.  Who is the contract between?  Generally you as the patient have an obligation to insure the medical provider gets paid.  Billing the insurance company is a courtesy to you and the most expedient way for the provider to get paid.  Fortunately I don't have to go to the Dr. often but when I have noticed in the paperwork I filled out and signs in the office that the patient is reponsible for payment.  Did the Medical provider send you notices?
