Question
after the first overtime period, how do they determine which team will be the offensive team first for subsequent overtime periods?

Answer
Don, hello!

They alternate possessions if both teams keep the score tied, until beginning with the third overtime, the team that scores a TD must also go for the 2 point conversion.  
They do not flip the coin again (only at the start of the  first overtime).
