I am a student in Colombia from the US and I currently have the cedula de extranjeria that is valid until December, at that time I will need to renew both my student visa and my cedula.  I have all the forms and documents for my visa ready to go.  Is there a form that is required to be filled out for the cedula.  I can not remember if I had to do paperwork last time or not.  I just remember waiting in line, going to the bank and waiting in line again so I want to have everything ready to cut down on the time I spend waiting!  lol   Thank you for your help.

Hi Michele,
Yes you do need a form, which you can get at DAS offices or at Subdirección de Extranjería in Bogotá. With the form you make the payment and then back to DAS to get the renewal.
