Question
Hi Claes,
I am 24 years old and have had found blood in the toilet after I use the restroom for the second time now… The first time I went to the doctor and I had upper GI x-rays done they found nothing… The doctor gave me some IBS medication and the bleeding stopped… 2 years later and it is back again… The doctor did a rectal check and sad he couldn’t see anything and because of my age I should just wait and see if it stops… He says it is probably a mild case of food poisoning and if it doesn’t stop he wants me to come back to see him… I have no pain fatigue or anything like that, my stools are very loose and the blood is bright red, sometimes looks like blood clots… The blood is not always there, sometimes I will go a couple of days without seeing any but it hasn’t stopped. I think I should just get a colonoscopy because I don’t think the doctor telling me your are young you shouldn’t worry too much is a proper diagnosis… What can be the possibilities with the symptoms I have??

Well. Since the blood is red it is most probably not from your stomach (stomach acid turns it black and tar like, so an upper GI study is most probably unnecessary (and that is MUCH better done by gastroscopy rather than X-rays by the way)). You also have loose stools. So that too points in the direction of your bowels, primarily your large bowel though I can not à priori rule out your small bowel. At your age colon (large bowel) cancers are indeed very rare (and small bowel cancers are rare in all age groups), but even so I had today (as you may be able to see for yourself here) a question from a young lady of 23 in New Zealand who already in spite of her age has had surgery for a colon cancer. So that it is rare does not mean that its risk is 0! Also there are other - even serious - diseases that are not that uncommon at your age that have similar symptoms, like ulcerative colitis and Crohn's disease. Both - especially ulcerative colitis - run with a higher risk of colon cancer too. You are quite right! You SHOULD at least have a colonoscopy done to make sure what this is! The doctor who will do the colonoscopy will have to decide if anything more is necessary. Please do keep me posted! Good luck!
